The safety in patients 6 to 23 months of age is supported by data from pharmacokinetic and safety and efficacy studies in asthma in this pediatric population and from adult pharmacokinetic studies. This associated weight loss may warrant evaluation and management of dysgeusia. Unintentional weight loss is common in the elderly and is a predictor for decline in activities of daily living, higher rates of institutionalization, and mortality.5 Depression is by far the most common cause of unintentional weight loss, followed by cancer, cardiac disorders, and benign gastrointestinal diseases. Medications that cause gastrointestinal side effects can also contribute to unintentional weight loss.6 Several medications have been implicated in the causation of metallic taste. Gastrointestinal tractrelated adverse reactions occur with all fluoroquinolone antibiotics.7 Furthermore, gastrointestinal disorders are the most common adverse reactions observed with use of levofloxacin 5.1% ; .8, 9 These reactions include diarrhea, nausea, flatulence, abdominal pain, dyspepsia, taste perversion, and vomiting. Chronic medical conditions such as chronic renal failure, antihypertensive medications such as valsartan, and gout treatments such as allopurinol are known to cause taste disturbances. Min Loke Wong Gout is an ancient disease, and was well known to Hippocrates. Using his invention, in 1679 van Leeuwenhoek observed microscopic gouty crystals. These crystals result from urate deposition in tissues, consequent on hyperuricaemia. They induce episodic arthritis, which initially are infrequent, usually affecting the foot, and respond well to anti-inflammatory medications such as colchicine and nonsteriodal anti-inflammatory drugs. However, these measures do not stop urate deposition from progressing. More frequent and widespread attacks may ensue, with increasing resistance to antiinflammatory therapy. Permanent joint damage may result from gouty erosions, and tophi formation may be complicated by ulcerations and sepsis. Thus, optimal management entails timely introduction of hypouricaemic therapy, to provide a gradient for resorption of the crystals. In New Zealand, allopurinol is the drug most widely used for lowering serum urate. In this issue of the Journal, Kumar and colleagues report their success in using another hypouricaemic drug, benzbromarone, as add-on therapy for patients with tophi and renal impairment, refractory to allopurinol.1 As the authors point out, gout is increasingly a therapeutic challenge in this country. However, readers of the Journal are more likely to encounter failed therapy with allopurinol due to suboptimal use, rather than the complex cases reported. Simply adding in, or substituting benzbromarone, is likely to also result in failure, if the factors underlying suboptimal therapy are not addressed. Hypouricaemic therapy is recommended when more than 1 or 2 attacks occur each year. A lower frequency may not motivate patients to adhere to treatment, to hopefully prevent polyarticular, erosive or tophaceous gout- which if already evident mandate such therapy. Allopu5inol loses effectiveness if taken noncontinuously, 2 by patients not understanding this, or mistaking it for anti-inflammatory treatment only to be taken during acute gout. The initiation of hypouricaemic therapy may provoke more frequent episodes of acute gout, especially when the disease is already advanced, discouraging patients from persisting with therapy. Using a low starting dose with subsequent monthly increases may be helpful, and prophylactic anti-inflammatory therapy is also recommended: in a recent prospective randomised double-blind controlled trial, the use of colchicine 0.6 mg twice daily reduced the number of flares during the first six months of allopurinol therapy, from an average of 2.9 per patient on placebo, to 0.5 per patient on colchicine.3 Starting hypouricaemic therapy during acute gout is inadvisable; however, should flares occur during hypouricaemic therapy, it is reasonable to continue such therapy alongside anti-inflammatory measures. Arterial blood analysis was performed after the intu bation. This probably explains the higher Po2 in group 2 on the first day. On the first day, the mean of the lowest measured values of Po2 is also given in Table 2, and it can be seen that these values were comparable between the two groups. The lack of correlation between Po2 and uric acid excretion probably reflects that Po2 does not correlate well with the tissue oxygenation, which is a key problem in the treatment of critically ill patients. This problem, in fact, is the main reason to look for other markers for tissue hypoxia, such as uric acid. In this context, we find it interesting that the values of excreted uric acid in group 1 were within the limits for healthy adults, which in a review was found to be 40 jimol kg 24 h range, 21 to 55 u, mol kg 24 h ; group 2, the excretion decreased toward normal limits during the study days. Five of the mechanically ventilated and one of the dead patients had very great uric acid excretion, more than 100 pimol kg 24 h, which was of similar magnitude as found in neonates with IRDS.5-6 In conclusion, our study showed that adults with acute respiratory failure requiring mechanical venti lation or leading to death had pronounced increased excretion of uric acid indicating increased degradation of ATP. The prognostic, clinical, and therapeutic implica tions need further study. We find several interesting aspects: The amount of excreted uric acid as an indicator of increased ATP degeneration may be a valuable sign of the severity of the tissue hypoxia and the efficacy of the treatment. In COPD, this could be of special interest, because the decision for mechanical ventilation in case of acute exacerbation is difficult due to the dubious prognosis. The clinical practice today is characterized by the lack of clearly defined criteria for start of and weaning from mechanical ventilation.
