We previously have described a nude rat model for melanoma metastatic to the brain using the human MRA 27 melanoma cell line Int J Radiat Oncol Biol Phys 28: 10791088, 1994 ; . The present study was designed to determine if Cereport mediated opening of the BBB could enhance the delivery of boronophenylalanine BPA ; and the efficacy of BNCT in nude rats bearing intracerebral implants of the MRA 27 melanoma. For biodistribution studies, BPA was injected intra-arterially via the internal carotid artery i.c. ; or intravenously i.v. ; at a dose of 500 mg kg body weight b.w. ; concomitantly with i.c. infusion of Cereport 0.1 g kg b.w. ; Peak tumor boron concentrations and the best composite tumor: brain 6.1: 1 ; and tumor: blood 6.3: 1 ; ratios were observed at 2.5 hours following i.c. infusion. At this time tumor concentrations were 65.4 19.5 g g with Cereport compared to 36.8 7.3 g g without it, and 19.5 6.2 g g following i.v. injection of BPA. For therapy studies, BPA 500 mg kg b.w. ; was administered either i.c. or i.v. with or without Cereport at a dose of 0.45 g kg b.w. over 10 minutes at 1314 days following intracerebral implantation of the MRA 27 melanoma. Animals were irradiated 2.5 hours later at the Brookhaven Medical Research Reactor with a collimated beam of thermal neutrons delivered to the head following which the animals were returned to Columbus, Ohio, where they were observed either until their time of death or euthanization at 180 days following tumor implantation. Rats that received i.c. Cereport and i.c. BPA had a mean survival time MST ; of 115 days range 59180 days ; compared to 82 days range 46180 days ; without Cereport, and 42 days range 2968 days ; for i.v. BPA with Cereport. Irradiated controls and untreated rats had MST of 31 5 days and 23 3 days, respectively. The physical radiation doses delivered to the tumor were 24.4 Gy for i.c. BPA with Cereport, 15.9 without it, and 4.4 Gy for irradiated controls and the corresponding photon equivalent doses were 85.3, 51.8 and 31.3 Gy. Our studies have demonstrated that the combination of i.c. Cereport and BPA can produce a 400% increase in the life span of rats bearing intracerebral melanomas with 36% long term survivors 180 days ; . Clinical studies are warranted to determine if Cereport can enhance the delivery of BPA for BNCT in patients with multicentric cerebral metastases of melanoma.
