The ability of various isoflavones to inhibit PDE appears to be isozyme-selective. We demonstrated previously that genistein was a mixed-competitive inhibitor of PDE4, and that inhibition was independent of tyrosine kinase activity Nichols and Morimoto, 1999 ; . In investigating the order of potency of the isoflavones, it became apparent that inhibition of PDE1 and PDE3 was similar, but for PDE4 was different. This suggests that the isoflavone binding site on PDE1 and PDE3 are much more structurally similar than that of PDE4. It is interesting to note that PDE1 and PDE3 can hydrolyze both cAMP and cGMP, whereas PDE4 is selective for cAMP. This substrate selectivity may be related to isoflavone inhibition. A summary of the structure-activity relationship of isoflavones is provided in Fig. 9. The hydroxyl groups on this structure are labeled for their importance in interacting with and ketotifen.

Clinical research shows that the soybean isoflavone has special curative effects to breast cancer, prostate cancer, heart disease, cardiovascular disease and to women climacteric syndrome. D. This trial showed reduction in frequency of hot flashes with all doses 2.2 fewer per day; 30%-58% vs. 19%, p 0.001 ; . Effects were greater for higher doses, but side effects more common. e. One study of gabapentin was rated as a good quality study. Breast cancer survivors were randomized to two doses of gabapentin 100 or 300mg TID ; or placebo and the 900mg day dosing showed a reduction in frequency of hot flashes compared to placebo 2.1 fewer per day; 44% vs. 15%; p 0.001 ; . f. Of the 4 trials of clonidine included in the meta-analysis, 3 used the 0.1mg day dose and 1 trial used a variety of doses 0.05-0.15mg day ; . 3 of the trials were rated as fair and one as poor. Eliminating the poor quality trial did not change the results. 5. Conclusions a. SSRIs SNRIs, clonidine, and gabapentin provide evidence of efficacy in reducing hot flash frequency. b. The frequency in hot flashes was not reduced for red clover isoflavones and results were mixed for soy isoflavone. c. After isoflavones are absorbed, a variety of the aglycone forms can be found in the plasma and the urine reviewed by Setchell ; .22 As discussed below, these agents have activities that may be important in treating or preventing BPH and prostate cancer. s POSSIBLE ISOFLAVONE MECHANISMS OF ACTION Effects on sex hormone synthesis and metabolism Isoflavones may play an important role in steroid metabolism and synthesis, thus affecting the proliferation of hormone-dependent prostate cells. 5-alpha reductase inhibition. Testosterone is converted to 5-alpha dihydrotestosterone, the main prostatic androgen, by the enzyme 5alpha reductase in the prostate. The isoflavones genistein and biochanin A are effective inhibitors of 5-alpha reductase activity in genital skin fibroblasts and in BPH homogenates.23 17-beta hydroxysteroid dehydrogenase inhibition. Another enzyme, 17-beta hydroxysteroid dehydrogenase 17-beta-HSD ; , also plays an essential role in the metabolism of androgens and estrogens. Genistein and biochanin A are effective inhibitors of 17beta-HSD activity in genital skin fibroblasts and in enzymatic assays conducted with steroid substrates.23, 24 UDPGT activation. UDP-glucuronosyltransferase UDPGT ; catalyzes the conjugation of steroid hormones with UDP-glucuronic acid, inactivates them, and facilitates their elimination from tissues. Abstract: Adhesion of mononuclear phagocytes Macs ; to extracellular matrices containing oxidized low-density lipoproteins oxLDL ; stimulates these cells to secrete reactive oxygen species e.g., O2, H2O2 ; that are believed to promote atherogenesis. Current in vitro systems designed to measure Mac H2O2 secretion in response to oxLDL show that these cells secrete H2O2 for only a few hours after plating. The slow onset and chronicity of atherogenesis, however, suggested to us that Mac ROS secretion might be sustained for much longer periods when Macs are maintained in an environment resembling that in the intima of arteries undergoing atherogenesis. The findings reported here confirm this suggestion. They show that Macs maintained on collagen IV matrices containing oxLDL in medium containing human plasma-derived serum secrete H2O2 continuously and in large amounts for at least 11 days. Using this system we tested the effects of compounds known to attenuate atherogenesis in vivo. Platelet-activating factor PAF ; receptor antagonists, lovastatin, and the isoflavone protein tyrosine kinase PTK ; inhibitor genistein each reduced H2O2 secretion by Macs maintained on oxLDL-containing matrices by 60%. Lovastatin's inhibitory effect was blocked completely by addition of geranylgeranyl pyrophosphate to the medium. Menopausal women suffer from deleterious effects of lowered estrogen levels including reduction of bone mass, menopausal symptoms, and hypercholesterolemia. Those effects are pronounced during early postmenopause because of drastic estrogen reduction, and tend to become attenuated during late postmenopause. Phytoestrogens can potentially alleviate hypoestrogenrelated deleterious effects. Isoflavones make up the most common form of phytoestrogens, and the major dietary source of isoflavones is soy. Asian people consume 10 100 times more isoflavones than Western people, 1 and osteoporosisrelated fractures are less frequent in Asian than Western communities, possibly because of the large quantities of phytoestrogen-rich soybeans and vegetables in the Asian diet.2 Various types and large amounts of soybeans and soy products are consumed especially in Japan, including soybean curd, fermented soybeans, soybean paste, and soy flour. Indirect evidence from studies of ipriflavone, 3 an isoflavone derivative, suggest a potential benefit of isoflavones in preventing osteoporosis. Other studies suggested beneficial effects of isoflavones on atherosclerotic disease and menopausal symptoms.4, 5 There has been a lack of epidemiologic studies on the relationship among dietary intake of soy products, menopausal symptoms, cardiovascular risk, and osteoporosis. The purpose of this study was to evaluate the relationship between intake of dietary isoflavones in soy products and menopausal symptoms, cardiovascular risk, and bone mineral density in postmenopausal Japanese women.

Determine the time from the moment of release until total recovery of the pink colour. If capillary refill takes longer than 3 seconds, check the pulse. Is it weak and fast? If the radial pulse is strong and not obviously fast, the child is not in shock. If you cannot feel a radial pulse of an infant less than 1 year old ; , feel the brachial pulse or, if the infant is lying down, the femoral pulse. If you cannot feel the radial pulse of a child, feel the carotid. If the room is very cold, rely on the pulse to determine whether the child may be in shock.

Chronic hemodialysis program of the University of Kentucky. The patients were dialyzed for 4 h three times weekly using cuprophane Gambro, Lund, Sweden ; , cellulose acetate Althin, Miami Lake, FL ; , or polysulfone membrane dialyzers Fresenius, San Francisco, CA ; . The dialysis surface area ranged from 0.9 to 1.8 m2, and the blood flow was 300 ml min with dialysate flow of 600 ml min. Table 1 reports the mean value of important clinical variables in the whole population, and Table 2 gives individual values in 11 of these patients who were randomly selected for further investigation involving intake of standardized amounts of isoflavone-rich soy protein isolate SPI ; . Additionally, 10 healthy subjects five men and five women, mean age 38, body mass index 24.1 1.9 ; volunteered for the study. None of the participants suffered from gastrointestinal problems that could interfere with isoflavone absorption, including acute gastrointestinal illness, the use of antibiotics within the past 3 mo, chronic malabsorption syndrome, chronic hepatitis, and liver cirrhosis. Furthermore, none of the participants suffered from decompensated heart failure, unstable coronary artery disease, respiratory illness, severe hyperparathyroidism, aluminum toxicity, cancer, drug addiction, ethanol abuse, or smoking of more than 10 cigarettes per day. All candidates were interviewed by a licensed dietitian. Participants were instructed to follow their routine diet with the exception of carefully avoiding isoflavone-rich food for 1 wk healthy subjects ; and 2 wk patients ; before their participation in the study. Informed consent approved by the University of Kentucky Institutional Review Board was read and signed by all subjects.
Sample stock solution from tablet formulation of 100 g ml was prepared.

2. Materials and methods Four isoflavones IF ; used in this work were extracted from the different Leguminosae species according to the procedures described previously: formononetin Sophora tomentosa L.; Komatsu et al., 1978 ; , irisolidone Sophora japonica; Komatsu et al., 1976 ; , licoisoflavone A Sophora moorcrofitana; Shirataki et al., 1991 ; , 6, 8-diprenylgenistein Euchresta japonica; Shirataki et al., 1982 ; . The chemical structures of the isoflavones used in this study are presented in Fig. 1. DPPC ; , brain phosphatidylserine PS ; , egg yolk phosphatidylcholine EYPC ; and yeast phosphatidylinositol PI ; were purchased from Sigma St. Louis, MO, USA ; . Lipids were used without further purification. PS and PI were delivered as chloroform solutions. Other chemicals used in the experiments were of analytical grade.

Industry's payroll. In 1992, Congress passed the Prescription Drug User Fee Act PDUFA ; , which required drug companies to pay user fees to the FDA, but stipulated that they would be used only to speed up approval of drugs. Many members of these committees have financial or other connections to interested companies. For example, three of the eight members of the FDA's Psychopharmacologic Advisory Committee, which recommended approval of Sarafem, reportedly had ties to Lilly. Soy Protein Isolates May Help Prevent Prostate Cancer Cancer Weekly -- July 31, 2007 -- Research findings, "Isoflavone-rich soy protein isolate suppresses androgen receptor expression without altering estrogen receptor-beta expression or serum hormonal profiles in men at high risk of prostate cancer, " are discussed in a new report. "The purpose of this study was to determine the effects of soy protein isolate consumption on circulating hormone profiles and hormone receptor expression patterns in men at high risk for developing advanced prostate cancer. Fifty-eight men were randomly assigned to consume 1 of 3 protein isolates containing 40 g d protein: 1 ; soy protein isolate SPI + ; 107 mg d isoflavones 2 ; alcohol-washed soy protein isolate SPI- ; 6 mg d isoflavones or 3 ; milk protein isolate 0 mg d isoflavones ; , " scientists writing in the Journal of Nutrition report. "For 6 mo, the men consumed the protein isolates in divided doses twice daily as a partial meal replacement. Serum samples collected at 0, 3, and 6 mo were analyzed for circulating estradiol, estrone, sex hormone-binding globulin, androstenedione, androstanediol glucuronide, dehydroepiandrosterone sulfate, dihydrotestosterone, testosterone, and free testosterone concentrations by RIA. Prostate biopsy samples obtained pre-and postintervention were analyzed for androgen receptor AR ; and estrogen receptor-beta expression by immunohistochemistry. At 6 mo, consumption of SPI + significantly suppressed AR expression but did not alter estrogen receptor-beta expression or circulating hormones. The petioles are fine, relatively short, 7-9cm, sparsely hairy, and green. Stipules are green with green veins. Calyx tube and lobes are green. Corolla white with distinct pink veins.
