Pergolide mesylate uspInhibition of -adrenergic receptors residing within these locations. -Adrenergic blocking agents have been used for more than a decade in the treatment of men with BPO and include terazosin hydrochloride, doxazosin mesylate, tamsulosin hydrochloride, and most recently, alfuzosin hydrochloride. -Blockade improves the symptoms associated with BPO but does not affect the natural history of the disease. Currently available -blockers are similar in clinical efficacy but differ in their treatment-related side effects. Prostatic growth is an androgendependent mechanism mediated by the 5 -reductase family of enzymes, which convert intraprostatic testosterone to dihydrotestoterone. Because of the progressive nature of the disease, men with enlarged prostates are at an increased risk of the development of complications from progression of their disease. The use of a 5-ARI in this subset of patients modifies the natural history of the disease and reduces the overall risk for acute urinary retention and BPO-related surgery. The 5-ARIs reduce the size of the prostate by reducing the amount of metabolically active intraprostatic androgens. The 5-ARIs currently available include finasteride and dutasteride. These two classes of drugs have been used as monotherapy or, most recently, in combination with one another with favorable results, as evidenced by a reduction in the AUA-SI score, increased peak urinary flow rates, and a delay in the natural progression of the disease. The choice of combination therapy is often dependent on prostate size and the presence or absence of irritative voiding symptoms and cefaclor. LODOSYN Carbidopa ; Upon first taking Sinemet levodopa carbidopa ; , some patients experience nausea and vomiting. For some, this problem can persist for months and prevent them from taking Sinemet as prescribed. By taking supplemental carbidopa pills, this problem can sometimes be effectively controlled. MOTILIUM Domperidone ; This medication has not been approved by the Food and Drug Administration in the United States. It is, however, available in Canada. Its primary value for the PD patient is in preventing levodopa-associated nausea and vomiting when extra carbidopa is ineffective. Preliminary studies would suggest that this is generally a safe medication and one that is well tolerated by patients. ZOFRAN Ondansetron ; This is a very expensive medication that is primarily used with cancer chemotherapy to prevent nausea. There are several brief reports of its value in relieving confusion, hallucinations and delusions in PD patients. In general, this medication is very well tolerated, with the major side effects being headaches, diarrhea and fatigue. The high cost of this medication, however, is frequently a prohibitive factor for treatment. HYDERGINE combination of ergoloid mesylates ; This agent was introduced many years ago for the treatment of senile dementia. Despite years of study, there is still controversy whether Hydergine improves thinking abilities and or behavior in those with dementia: Some of the agents contained in Hydergine have dopamine agonist properties and have been shown in both human and animal studies to have anti-parkinsonian effects. The doses used in these studies are, however, many times those customary for Hydergine use in medical treatment. Even at high doses, the ergoloid mesylates have not been directly compared to available dopamine agonists, such as pergolide, to see if they have any special advantages. Hydergine and its related compounds are claimed to have anti-oxidant properties, but the clinical significance of these effects is unknown. Side effects of treatment include nausea, stomach upset, headache and low blood pressure. At high dose, these agents may also be associated with spasm of the blood vessels in the arms or legs. BOTOX Botulinum toxin A ; This is one of the most potent biological toxins known to man. When injected into a muscle, botulinum toxin poisons nerve endings, causing muscle weakening and wasting, which can persist for several months. Some patients with Parkinson-related dystonia can have the severity of their dystonia reduced by receiving injections into the dystonic muscles. Treatment with this agent is very expensive. FLORINEF Fludrocortisone acetate ; This is the most commonly used medication to treat symptoms of orthostatic hypotension, which is a severe drop in blood pressure caused by standing. Florinef causes the body to hold onto salt and water and can increase the volume of fluid within the circulatory system. Ankle swelling and weight gain are expected side effects of treatment. Persons receiving Florinef should cautiously increase their salt intake to enhance its actions. Potassium supplements are additionally indicated in many patients to prevent potassium levels from falling dangerously low.
