Methods: Over a 6-month period, 55 aneurysmal SAH patients underwent monitoring for cerebral vasospasm using twice daily TCD and continuous transcranial cerebral oximetry rSO2 ; . Using the INVOS 4100 monitor, a relocatable headband with a near-infrared light source and sensor pads were placed bilaterally over the forehead, representing the anterior watershed of both hemispheres. Patients were monitored from post-SAH Days 3 to 14, with measurement recorded onto 3.5" diskettes and compared with twice daily TCD. Results: Twenty of 55 patients developed evidence of cerebral vasospasm by TCD and rSO2 criteria. Two additional patients had evidence of advanced vasospasm by rSO2 criteria, with normalization of TCDs and confirmed by cerebral angiography. Eight patients underwent cerebral angioplasty for medically intractable cerebral vasospasm when immediate changes in rSO2 could be observed. The pattern for vasospasm as indicated by rSO2 monitoring included: rSO2 40, postural-dependent rSO2, and an erratic pattern of rSO2 characteristic of flow-metabolic uncoupling. Conclusions: The cerebral oximetry provided reliable and consistent data which correlated with TCD and cerebral angiography in both the detection of delayed cerebral vasospasm and therapeutic intervention.

From the standpoint of the diagnosis of ADHD, it is important to ascertain whether the symptoms were present at a young age, even if they were never formally evaluated. Absence of any symptoms prior to high school or college suggests an alternate diagnosis. The Diagnostic and Statistical Manual of Mental Disorders, Fourth Edition DSM-IV ; criteria required for a diagnosis of ADHD are listed in Table 3.8 Clear evidence of clinically significant impairment in social, academic, or occupational functioning is essential for an ADHD diagnosis according to DSM-IV.8 A recent study reported that approximately 4% of 448 students surveyed at a Midwestern college met criteria for ADHD using items from the DSM-IV criteria on a selfreport instrument.9 When more lenient cut-offs were used 4 symptoms rather than 6 in either ADHD dimension [eg, inattentive or hyperactivity impulsivity] ; , up to 11% of the study sample met the criteria for ADHD.9 The AACAP Practice Parameters document notes that when taking a student's medical and psychiatric history, close attention should be paid to neurological conditions eg, head trauma, seizures ; and anxiety, depression, bipolar, and substance abuse dependence disorders Table 4 ; .7 Prior experience with psychotropic medications should also be documented. A thorough family history is required, as ADHD is most often a familial condition, similar to learning disabilities and psychiatric conditions. Psychiatric functioning should be assessed with a systematic review of symptoms and current mental status. Finally, it is important to determine what beliefs the student has about his or her difficulties, what expectations they have regarding treatment, and their level of preparedness to take responsibility for their care.
