Early in our investigations of malformations in the northern leopard frog it became clear that the malformations were the result of teratogenic events occurring during early developmental stages. With this insight, we began an investigation in 1998 examining tadpoles at sensitive stages of development. As part of this two-year investigation, tadpoles just beginning metamorphosis Gosner stage 40 to 43 ; were collected and examined. Of the 28 tadpoles collected from one Wisconsin site over ten days, 15 had facial clefts involving degeneration and loss of the suprarostral cartilage. Approximately 2, 200 tadpoles have been examined from 15 sites in 4 states and this malformation has not been seen elsewhere. Microscopic examinations of affected tissues showed degenerative changes consistent with apoptosis. Apoptosis is genetically programmed cell death critical to normal remodeling of tissues during development. Facial clefts have been documented in tadpoles of Xenopus laevis and Rana temporaria experimentally exposed to DDT as well as Xenopus laevis tadpoles exposed to corticosteroids. Experimental studies in mammals have also produced facial clefts including cleft lip and cleft palate ; by exposing the developing fetus to; vitamin A; jervine, a steroid alkaloid from the toxic plant Veratrum californicum; toxic levels of ethanol; methyl mercury; hyperthermia and others. This is the first report of spontaneously occurring facial clefts in free- living tadpoles, for instance, why was zelnorm taken off the market. Title 22 emphasizes the need for maintaining working relationships between referring perinatal units and intensive newborn nurseries. Communication between providers is critically important as is the provision of education. Title 22 stresses the importance of the receiving unit providing joint staff conferences and continuing education for perinatal and neonatal care. Regional Cooperation Agreements RCA ; must be in place for all hospitals who have neonatal intensive care units NICUs ; designated by the California Children's Services CCS ; as either an Intermediate, Community or Regional Center. The RCA is one component of the CCS standards which impacts quality of care by requiring 1 ; joint education and training of perinatal health professionals, 2 ; joint development of guidelines for consultation by perinatal, neonatal and other specialty disciplines as indicated, 3 ; joint development of guidelines for maternal and neonatal patient referral and transport to and from each facility NICU, 4 ; joint identification, development and review of protocols, policies and procedures related to the care of high-risk obstetrics and neonatal patient, at least every two years and 5 ; joint review of outcome data at least annually. Some hospitals will have many `agreements' in place to ensure the availability of quality services for their patients.
